Choice Therapy wants you to understand your insurance and the financial implications of treatment. Keep in mind that each policy for each insurance company is different, so please call your insurance company to find out the following details:
- Does your insurance cover PT/OT/Speech?
- Do you need a physician referral for treatment?
- Is Choice Therapy an in-network or out-of-network provider for your insurance policy?
- What is the dollar amount of each of the following?
Co-Pay
This is the dollar amount you will pay each visit (Due at the time of visit)
Deductible
This is the amount you pay before insurance pays anything in a calendar year. This is usually per person with a maximum out of pocket per family per year.
Co-Insurance
This is the percent you will pay after insurance has paid the allowable amount for services. This is different for each insurance company and each insurance policy.
What is Same Day Pay and When would this Option Benefit You?
Same Day Pay is an option for all patients, except those with Medical Assistance, Medicare or Medicare replacement plans as this is not allowed by the state and federal governments. Same Day Pay allows you to pay for your services directly at the time you arrive for your appointment(s).
What is a ‘Good Faith Estimate’ and do I qualify?
Under the law, health care providers need to give patients, who don’t have insurance or who are not using insurance, an estimate of the bill for medical items and services.
Transparency in Coverage
Follow this link to access machine-readable files (MRFs) that are made available in response to federal Transparency in Coverage rules. The MRFs contain pricing information about covered items and services, and are formatted to allow third parties to access and analyze the data.
What is the cost for Same Day Pay?
- $115 for the initial visit
- $90 for each additional visit
When may Same Day Pay be a better option?
- If you have a high deductible that has not been met in the calendar year.
- If you do not have insurance.

HOW DO I PAY FOR THERAPY?
Your health insurance provider usually covers Physical Occupational and Speech Therapy services. Several insurance plans have either a deductible or copay. It is best to check with your insurance provider regarding coverage and limitations to rehab services. We also provide several payment options to you, including same day pay and credit card payments. Give us a call and we will discuss the best options for you.
ACCEPTS ALL INSURANCES
Because we are independent and locally owned, Choice Therapy accepts all insurance companies. Your health insurance provider usually covers physical, occupational and speech therapy services. Several insurance plans have either a deductible or copay. Some insurance companies do require a referral to qualify for their particular insurance plan. It is best to check with your insurance provider regarding coverage and limitations to rehab services; or our staff will gladly assist you with this question.
DIRECT ACCESS
The ability of a person to see a therapist without a physician’s referral.
A physical therapist is able to evaluate and treat a patient for up to 60 days without a referral. Keep in mind deductibles, co-pays and other health care costs your insurance company require, would still apply. It is recommended to contact your insurance company regarding “Direct Access”; or our staff will gladly assist you with this question. Simply contact our clinic and we will set up an evaluation with a physical therapist.
CREDIT CARD PAYMENT
We accept all major credit cards for payments including same day pay, co-pays, and deductibles.
SAME DAY PAY OPTION
As our health care system changes, many of us are opting for high deductible plans or no insurance coverages at all. Choice Therapy offers same day pay option for people with this type of scenario giving you the power to see a therapist, the freedom to set the proper visit limits with your therapist and pay a flat rate each session without billing your insurance company.
